Student Advocacy Campaign Form
This form must be completed by a staff supervisor responsible for the student group or student interested in hosting an awareness-raising campaign or awareness-raising combined with fundraising initiative.

For Donation Drives only:
Got it! The teacher supervisor will email FacMan and the students will prepare the A4 page for each box
If you are hosting a donation drive, please note the following: - The drive can last for as long as 1 week. - Use only the red donation drive containers (ask your teacher supervisor to contact FacMan at least 2 days in advance to arrange use) - The containers can be placed in the Level 9 main lobby, Level 7 near the bus area and the Level 5 Forum. - Groups are responsible for placing an A4 sized paper in the plastic sleeve on the box to tell everyone what the drive is collecting and who are the beneficiaries.
To reduce our environmental footprint, please tick and keep in mind the following:

use only recycled paper, 1-sided paper or used cardboard as signage for the event
for food sales: no disposable cutlery provided to customers (no straws, plastic bags, etc.)
for food sales: all food & drink vessels & packaging MUST be made of materials that can be recycled or reused
